Direct Primary Care Plan Pricing*

Current Membership Fees are:

$150/month per adult (ages 24 and up)

$100/month per adult (ages 18-23 years)

$55/month for children age 0-17 (when joining with an adult membership)

$100/month solo child 0-17 years (not joining with adult membership)

Discounts Available: 

10% off for veterans, health care workers, and first responders

10% off Family Plans (3+ members)

Travel Fees for house-call and mobile visits:

- Addresses within 5 miles of Practice*                                     $50

- Addresses 5-10 miles from Practice*                                       $75

- Addresses 11-25 miles from Practice*                                     $100

- Addresses located >25 miles from Practice*                  $3.44/mile from Practice*

**  Please contact our office regarding appointments >40 miles from practice

Resort Fee of $25 will be added to visits which take place at a hotel or resort.
